Liability Release

By registering for this tournament, you agree to the following liability release:

I acknowledge and fully understand that I may be engaging in activities that involve risk of serious injury, permanent disability, or death, and severe economic losses which might result not only from my own actions, inactions or negligence of others, the rules of play, or the conditions of the premises or of any equipment used.

 

Further, that there may be other risks not known to us or not reasonably foreseeable at this time. 

 

I assume all the forgoing risks and accept personal responsibility for the damages following any injury, permanent disability, or death. 

 

I do hereby release, waive, discharge and agree to not sue any OMEGA MARTIAL ARTS Corporations, OMEGA TAO FEDERATION, LLC., or MOUNTAIN VIEW HIGH SCHOOL and/or APLINE SCHOOL DISTRICT, its affiliates, from ALL liability to the undersigned.
