COVID-19 Protocol: At the Shine On for Shihan Tournament the safety of our athletes, officials, staff members, volunteers and spectators is paramount. At this time Utah State does not have any restrictions in place, except the standard "if you are sick, please stay home". Protocols may change if additional restrictions are put on by the convention center, state, or local health departments.

If State restrictions change before the event, the Shine On for Shihan Tournament reserves the right to require masks. For example, we may need to follow USA Taekwondo guidance which is as follows: Masks will be required for all individuals regardless of vaccination status within the tournament and event space. Athletes will not be required to wear a mask during competition in the field of play. Coaches and officials will be required to wear masks during competition including field of play.

I acknowledge and fully understand that I may be engaging in activities that involve risk of serious injury, permanent disability, or death, and severe economic losses which might result not only from my own actions, inactions or negligence of others, the rules of play, or the conditions of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

I do hereby release, waive, discharge and agree to not sue any OMEGA MARTIAL ARTS Corporations, OMEGA TAO FEDERATION, LLC., or MOUNTAIN VIEW HIGH SCHOOL and/or APLINE SCHOOL DISTRICT, its affiliates, their respective administrators, officers, directors, agents, coaches, and other employees or volunteers of the organization, other competitors, owners and leasers of the premises used to conduct the event (all of which are referred to as “releasee”), from ALL liability to the undersigned, his or her heirs and next of kin for any claims, demands, losses, or damages, on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the release or otherwise in connection with the association or entry in and/or arising out of my travel to, participation in, and returning from this competition.
THE COMPETITOR HAS READ THE ABOVE WAIVER AND RELEASE AND UNDERSTANDS THAT HE/SHE HAS GIVEN UP SUBSTANTIAL RIGHTS BY VOLUNTARILY SIGNING UP FOR THIS TOURNAMENT. (If Competitor is under 18, Parent or Guardian must agree to this liability release)
